Answer: 1.1 Person centred practice is the personalised care that an individual receives, this is written with the service user, their family, friends, social worker, carers, advocate and anyone else who plays a significant role in their lives, It is to ensure that the service user receives care that is centred around them, that the care and support they receive is for them and not that they have to change or adapt to the service that is provided. As a manager I would assess any referral to make sure that we could meet their needs. The needs of the service user are monitored to ensure their needs are being met. There are situations when the support plan, risk assessments and behaviour support plan need to be reviewed if there is an incident and their needs change. All staff, family members and the service user are made aware of this.
