Answer: Working in partnership is a key element within the care setting, ensuring each individual is receiving the care needs they require, working with service providers as a home we can ensure all these care needs are being met.
By effective partnership we can achieve better outcomes to be able to deliver the best care. Information that is shared is for the benefit of each resident, by partnerships working together and gathering all the information together builds a bigger picture of each resident, a more person centred care package can be delivered within the care setting. Each partnership may see things differently, so by all working together they can implement and plan for all the needs not just one that may be seen by one professional.
